Narcissism and Its Vicissitudes

The current sociopolitical environment has intensified public interest in narcissism and its manifestations. It is well known among clinicians that clients with narcissistic personality disorders are among the most challenging to treat due to their tendencies to externalize blame and flee treatment prematurely. Over time, psychoanalysts have conceptualized narcissistic disturbances in various ways, with differing ramifications for intervention. Steven Ellman’s article describes the narcissistic defenses as conceptualized by psychoanalytic theoreticians from Freud to present. Otto Kernberg presented technical interventions for narcissistic transferences as reflected in distortions of free association. Finally, Glen Gabbard and Holly Crisp-Han’s book chapter explores the transference and countertransference that often arise in treating patients with narcissistic disorders.
